I think you have to assume that the caption is set in a (near-future?) world where people are a lot less uptight about gender than they are in ours; tricking a guy into becoming a woman for a year - and being able to make a living from doing not-particularly-much work - is probably seen more as a mean prank than anything genuinely sociopathic. Remember, he can quit whenever he wants....
