Though the backstory seems to be missing. What happened to him/her? Who is they? The therapist? Some assholes who kidnapped him and turned him into a girl?
Yeah, it's not a good place for her right now, is it? Sometimes it's good to just focus on a particular detail - how a character copes with a situation, how they think of their new body, the idea of self-acceptance as surrender... The back-story isn't that important - you inferred all that was necessary. Somebody, for some reason feminised him against his will. Perhaps his drink was spiked one night. Perhaps he was kidnapped by some weirdo group of cultists or political extremists. Perhaps his country has a government with a policy of feminising people for some reason. At times like this, the reader can imagine their own backstory; they'll probably come up with one they like better than the one I come up with. :) Anyway, I'm glad you liked it!
